To obtain a health insurance card for any reason:
- If you are enrolled in COVA Connect, call Optima at 687-6350. On initial enrollment, a card is mailed to the employee’s home address for each enrolled family member. New cards are issued when a family member is added to the employee’s coverage.
- If you are enrolled in COVA Care call Anthem’s ID Card Order Line, 866-587-6713. On initial enrollment, the employee is mailed two cards. Cards have the employee’s name on them. Cards are not personalized to each enrolled family member. New cards are not issued when a family member is added to the employee’s coverage.
