Prior to the birth, contact Human Resources to determine your eligibility for the Family & Medical Leave Program and if enrolled in the Virginia Sickness & Disability Program (VSDP) contact the VSDP administrator, UNUM at 800-562-5602. When you call UNUM, have available your doctor’s name, address and phone & fax numbers.

You have 60-days from the birth or date of adoption/placement to: (You will need a copy of the hospital provided “Proof of Birth Letter” or adoption/placement for adoption paperwork)

+ Add the new born to your health coverage.
+ Enroll in or change your coverage (add or remove optional features).
+ Cancel your coverage to go onto your spouse’s coverage. You will need documentation that you will be covered by your spouse’s coverage.
+ Enroll in or increase your Medical Reimbursement Account.
+ Enroll in or increase your Dependent care Reimbursement Account.
+ If your Medical and/or Dependent Care will be covered under your spouse’s coverage, with documentation, you may cancel your Medical and/or Dependent Care Reimbursement Accounts.

Once the new born has been issued a Social Security Number, please provide the number to the Benefits Office.

Consider reviewing your beneficiary designations for your Basic and Optional Life Insurance, Tax Sheltered Annuity (TSA/403b), Cash Match (CM/401a) and/or Deferred Compensation Plan (DCP/457) accounts and your VRS or ORP retirement account.
