You have 60-days from the divorce being final to remove your former spouse: (A copy of the final divorce decree will be required)

+ You must remove your former spouse and formerly covered non-custodial, step-children from your health coverage.
+ Change your coverage (add or remove optional features).
+ Enroll in coverage if you were previously covered by your spouse’s health ins.
+ Enroll in or increase your Medical Reimbursement Account when coverage under your former spouse’s plan is lost
+ Reduce or cancel your Medical Reimbursement Account.
+ Enroll in, increase or reduce your Dependent Care Reimbursement Account.
+ Cancel your Dependent Care Reimbursement Account if you are not the custodial parent.

Legal Separation is not a divorce – the above options are not available when you get separated.

Consider reviewing your beneficiary designations for your Basic and Optional Life Insurance, Tax Sheltered Annuity (TSA/403b), Cash Match (CM/401a) and/or Deferred Compensation Plan (DCP/457) accounts and your VRS or ORP retirement account.

If you are going to change your name, obtain a new Social Security Card and provide a copy to HR.

Provide HR with a mailing address for your former spouse. This is address is required to provide the former spouse with COBRA Health Care Extended Coverage information.

Provide Human Resources, your TSA, CM, DCP and/or ORP retirement company(s) with any new mailing address.
