If you are:
+ expecting a baby,
+ planning to adopt,
+ have a serious health condition which will require you to miss work,
+ have an immediate family member with a serious health condition who will require your assistance,
Notify your supervisor and Human Resources 30 days in advance or as soon a practicable.
There’s also a program which provides for similar protection to care for an injured military service member or to address certain qualifying exigencies resulting from active duty or call to active duty of a National Guard or Reserve service member.
Human Resources will assist you with determining your eligibility for Family Medical Leave. Family Medical Leave is a program which provides employee’s with up to 480 hours/12 weeks of job protection during qualifying absences.
Absences under the FMLA must be covered by the use of a paid leave unless you do not have paid leave available, then the absence will be leave without pay.
If the issue is birth or absence for your own health condition, employees covered by the Virginia Sickness & Disability Program (VSDP) should, in addition to contacting Human Resources, contact the VSDP administrator, UNUM at 1- 800-652-5602 to activate their VSDP Short-Term Disability income protection benefit. When you call UNUM, have available your doctor’s name, address and phone & fax numbers.
For birth of a baby, you have 60-days from the date of birth to add the baby to your healthcare plan.
