Required Information





The Transcript Request Form includes all the information we need to fulfill your request. If you submit a written request, please be sure to include the following information:

  1. your name (please include middle and maiden name) (REQUIRED) -- the rest is to help us fulfill your request and to ensure that the correct record is used. If you attended ODU under another name, please include that information. 
  2. your date of birth
  3. your University identification number (or social security number)
  4. your dates of attendance at Old Dominion University
  5. your permanent address
  6. an email address and/or daytime telephone where you can be reached
  7. the complete address to which the transcript is to be sent
  8. your payment or payment information (see Fees)
  9. your FedEx account number for overnight/express mailing
  10. your signature (also REQUIRED).

Missing information may delay the processing of your request.

Please use a separate form (or request) for each address to which transcripts are to be sent.

NOTE: If you want your transcript sent by overnight (Federal Express), you must provide a personal FedEx account number. More information can be obtained at www.fedex.com.
